- TB - Michigan/MIOSHA policy - N95 Annual fit-testing starts in new fiscal year:
The Fiscal year started October 1, 2008 without a signed FY08 budget, but President Bush did sign the appropriations bill (HR 2764) funding the government on Dec 26, 2007. The budget no longer contains language restricting the use of federal dollars for annual fit testing enforcement. Due to prior uncertainty, MSIC had urged members last October to begin the change to annual fit-testing for TB immediately by updating policies and implementation plans for annual fit-testing. MIOSHA has the authority to enforce the rule. MIOSHA Revised TB Directive based on CDC 2005 TB Guidelines
In June 2007, MIOSHA posted a revised TB enforcement directive adhering closely to the CDC 2005 TB Guideline. It recognizes flexibility in TB testing based on risk assessment. However, the checklist also includes the requirement for initial and annual fit-testing, following federal OSHA directives. This change applies to FY2008 since the federal budget was approved December 2007 and no longer contains language that prohibited enforcement of annual fit-testing. MIOSHA is now enforcing the OSHA's revised Respiratory Protection Standard Respiratory Protection. - 1910.134.

Centers for Medicare & Medicaid Services (CMS) Issues Guidance on Use of Alcohol-based skin products:
i) CMS directive A letter to all state surveyors supporting the use of alcohol-based skin preps in all surgical settings, based on NFPA fire prevention strategies has been issued by CMS. For details click here
ii) Alcohol-Based Hand Rub (ABHR) gel and foam Updates. CMS considers gel and foam hand rub dispensers in the corridors in healthcare facilities as equivalent and allowable as long as these comply with their final rule. For details see: http://www.ashe.org/ashe/codes/handrub/index.html

Please note: Michigan has revised its policy on the distance between dispensers and electrical outlets. The distance is not defined but fire marshals will judge safe distances based on evidence of splashing near the outlets. Michigan is aligned with CMS on this issue. -
APIC & SHEA Issue New Position Paper on Topic of Legislative Mandates for MRSA Surveillance:
The Association for Professionals in Infection Control and Epidemiology, Inc (APIC) and Society for Healthcare Epidemiology of America (SHEA) have convened a working group to review the potential impact of legislative action aimed at preventing the spread of MRSA and VRE. Full text of this available from the link below but consensus was that a legislative mandate is inappropriate because it would not allow flexibility to respond to changing local epidemiology.

The folder below provides a number of tools to assist with surveillance of central line associated bloodstream infections (CLABSI).
